Horacio T. Templo (HTT) retired from the Social Security System end of the year 2010, the year Emilio S. de Quiros was appointed by Philippine President P-Noy Aquino.
HTT was Chief Actuary and EVP of Branch Operations at the time. he was often in the news because contribution and pension increases are always supported by actuarial studies. as head of Operations, procedures in the branches had to be approved by him.

Hi Mam, i do have a question. I am from Comben department of our company. We submitted documents to SSS Aug 2014, and the said that the approval was sent to us Sept 4, 2014. March 2015, we haven't received anything from SSS that's why we communicate with SSS that we haven't received anything from SSS. We were advised by SSS that we need to submit certification letter stating that we did not received anything approval papers from SSS. we tried to ask SSS for the registry number of the letter so we can traced it. They said that they were not providing it. So, what we did is to submit a certification letter, we received it June 25, 2015 and submitted to SSS July 18, 2015 instead of the July 11, 2015 which is the due date. Is there any way for the company to dispute on this? to extend the date of the claims since we received the new approval form June 25, 2015? Thanks a lot.

DeleteMs guia goodeve po ask KO lng Na fracture kasi 2 paa KO Na cast po pareho. Dec19 po nagyari. Then sabi po ng Dr. SA otho. Till Jan 13 po ung cast so ginamit KO nlng po lahat ng sick leave KO but Pag balik nmin sabi po ng Dr.Hindi po ng heal so extended till Feb 17 Daw po. So kinabukasan KO po nlaman ng file Na po ako SA office para maipasa SA sss. With medical cert.at medical abstract. So ask KO lng possible po ba maapproved kahit ung extended days of confinement KO nlng po? Ng woworry po kasi ako about 5 days policy ng sss. . Pasok sya SA 5 days if ung extended Na. Please help me 1st time KO po kasi so I don't have enough idea. Thanks godbless
ReplyDeleteHi maam I wanted to apply for sss sickness as separated employee.sss needs to have the cert of employment from prev company however they will not be able to release it yet since I just resigned and they can release it after 2-3 months from resignation date.but prev company said they can provide a certificate of contribution they said they will just sealed it.Maam is it possible to submit cert of contribution and cert of non cash advance?since I cant have my cert of employment yet will that be accepted?:(

ReplyDeleteHi! I passed all the qualifications to file for sickness benefit except for 1- i still have a day remaining sick leave. This i was not aware and definitely a miss on my part. But will my application be considered since i was advised by my doctor to rest 15 days?
ReplyDeleteTanung q lang po pwd ko ba ecomplain yung egency ko kasi yung sick leave ko aprove na po ng sss kaso almost one month na po hindi na nila nirelease yung checky
