Qualifying conditions:
- paid at least 3 months within the 12-month period prior to the semester of contingency.
- must not be the fifth delivery/miscarriage (only 4 are covered)
- member must have notified SSS at least 60 days from the date of conception
Requirements for all filers:
- Maternity Notification duly stamped and received by SSS at least 60 days from the date of conception.
- Maternity Benefit Reimbursement Application form duly accomplished
- for a normal delivery, certified true or authenticated copy of an NSO birth certificate
- for a Caesarean delivery , certified true and authenticated copy of an NSO birth certificate AND certified true copy of operating room record/surgical memorandum
- for normal or Caesarean delivery for cases where the child dies or is still born, an NSO death or fetal death certificate must also be submitted
- for miscarriage or abortion - dilation and curettage (D & C) report for incomplete abortion, pregnancy test before and after abortion WITH age of gestation or histopath report for complete abortion
Additional requirements for Separated members:
- certification from last employer showing effective date of separation from employment OR notice of closure or strike of company OR certification from DOLE (Dept of Labor and Employment) that the employee member or employer has no pending labor base
- certification that no advance payment had been granted - if confinement days applied applied for are within or prior to separation from company
Additional requirements for SE/VM (Self Employed/Voluntary Members)
- xerox copy of SSS Form RS-1 (Self-employed Member Application).
- xerox copy of SSS Form E-5 (Voluntary Member Application) or RS-5 showing VM status

hi can i still have my SSS maternity benefit recomputed? My employer already gave me my SSS benefit before I take my leave and before my end of contract day. They based it through SSS online computation of benefits. Sadly upon checking with my SSS online they just reimburse only half of what they gave me so mean to say I still owe my employer now.

Deletehi maam im roma magandang gabi po ask ko lang po if mkakapag avail pa ba ako ng maternity benefit ko last dec. 2014 po ung last hulog ng employer ko tapos balak ko po sana icontinue hulog ko as VM this year ang expected date of delivery ko is april 2,2016 kung babayaran ko po ba yung january to march ko maaavail ko po yung maternity benefits ko di po kc ako nkahabol sa payment nung january 20 for oct. to dec. 2015 pano po kaya yun maam??thanks po in advance
ReplyDeleteHow can you submit histopath report for complete abortion eh completed na nga ung abortion and you don't know kung saan at kelan lumabas ung dugo.. I already submitted the ultrasound report before and after dp ba un pwede?
ReplyDeletehi maam, last march 2015 po ang last contribution ko,Ive start working po and paying my contribution ng june 2014 then until now d napo ako naka hulog dahil hindi po nag dededuct yung employer ko, naisipan ko po mag voluntary pay nalang. 1 month pregnant po ako and nag woworry po ako na baka wala ako makuha na maternity benefit. Need ko papo ba hulugan yung mga time na hindi ako naka hulog?? thank you!
